Yes, you would start the questions on the page after the title page.
When instructors say that they want your answers "in APA format" they mean that they want you to use the basic structure of crafting an APA paper. So they expect you to have these things:
- 1-inch margins all the way around
- Font size - 12 point
- Font - Times New Roman style
- A title page
- A References page (when appropriate) in APA citation style
As you are not writing a paper, you would not need an Abstract.
