How do you formulate a questionnaire in APA style?

Use the general APA formatting rules for the questionnaire:
- One of the following fonts:
- Times New Roman, 12 point size
- Calibri, 11 point size
- Arial, 11 point size
- Georgia, 11 point size
- Lucida Sans Unicode, 10 point size
- Computer Modern, 10 point size
- Double-spaced
- For more information, visit the APA Help guide.
- A questionnaire used in research can be included in an Appendix.
Other than the above guidelines, there is no specific format in APA for writing questionnaires.
